1992 Lexus SC 400 4.0L MIL Lamp on
Thanks to all that have responded. As suggested, I tested for air leaks and MAF problems. Eveything looked good, so I tried to sell an ECU. The customer declined repair, they had been trying to decide on a new car anyway, so this may be the last straw.

2005 Subaru Outback 2.5L Heater
Thank you for all that replied. So the Fix is to do mulitable heater core flushes. The coolant always looked clean but the flushing seems to has cured the problem. Customer has called back to say all is A-OK.
