I recently purchased a 2003 Chevy Astro AWD with the 4.3 liter engine. It has a faint rattle under acceleration at around 2000 rpm. Alldata shows that there was a bulletin from GM that says the noise could be caused by torsional vibration of the balance shaft.... Login to read more.
