Hey Michael , I only had it happen to me once on a Nissan Quest but have you ever had a flash fire ? I had two problems a lean code and a misfire code. Guess what, a plug wire boot was arcing and I had a vacuum leak. The flash fire only lasted about 2 seconds... Login to read more.
