I had a G van with 5.0M motor do similar. Bad dist cap at the 3 and 5 area. The two were cross firing. If low enough RPM the engine would kick back and stall kinda violently. I see you didnt try a cap yet. It would certainly explain your misplaced firing event... Login to read more.
