We had one here a week or so ago with a similar issue. 4,5,6 dead failed cut out test passed everything else. Swapped 4,5,6 with 1,2,3 same thing 4,5,6 dead. Turned out to be injector drive module was shot, new module programmed and good to go. Login to read more.
