It's a mix of gelled coolant, and trash in the system clogging the coolant flow through the cooler.

Check EOT vs ECT, they should be within 25 degrees per Ford, but many like to see it closer. If the EOT is higher, the EGR temps are probably sky high as well... Login to read more.
