I've been wearing the Uvex...the numbers on the replacement lens bag are:

6561

ASTRO OTG 3001 (this may be the number of the glasses)

1 EA 5104 (may be the lens #)

[...] [[...]]

is where I get them.

They're OK...fit fairly well.
