As Rex said, dig into that EBP sensor (Exhaust back pressure...)

They DO use that to monitor for misfires (although I don't believe it's the only stragegy, but that's speculation/interpolation on my part...). The ECM is watching the "pulses" as each cylinder... Login to read more.
