2001 Ford F-250 Super Duty Lariat 7.3L Drive VIN: 1FTNW21F01
Upon further inspections, I removed the pan and found they had installed a shift kit and had the spring for the accumulator on the wrong side of the piston. installed spring correctly and all is well.

Thanks guys you were all pretty much on the money. After clearing codes & test drive with modis data scan, it was perfectly clear that sensor #3 was faulty. all sensors should reach 1000-1400F to burn off soot & turn it into ash during regeneration.
