Upon further inspections, I removed the pan and found they had installed a shift kit and had the spring for the accumulator on the wrong side of the piston. installed spring correctly and all is well.

After clearing codes & test drive with modis data scan, it was perfectly clear that sensor #3 was faulty. all sensors should reach 1000-1400F to burn off soot & turn it into ash during regeneration.
