It cracked because the threads are tapered and a harder fitting was overtightened. A little Teflon tape would easily prevent this.

Most items tapped for 1/8 NPT are tapered. It is actually difficult to find a 1/8 NPT or any size pipe tap that is not tapered.... Login to read more.
