Are you sure you are not shifting 1-2-l/u? Instead of 1-3-4? I have seen stripped sunshells cause this and still have reverse. It is rare,,but it has happened here on more than one occasion. Also a band broken or unable to apply will kill 2nd and 4th. Login to read more.
