FYI: The Mass Air Flow Sensor was indeed the problem. It was a constant 0V, 0 grams/sec. And the PCM was in limp in mode, no 4th gear. Just never had a MAF go bad without other driveability issues. Lesson learned. Normally I would have just fixed the MAF issue... Login to read more.
