Though I don't work on these, a couple of things come to mind.

You mentioned sampling the exhaust at the O2 sensor hole. If the O2 sensor is removed from the exhaust stream, the O2 sensor is going to "see" a lot of oxygen, (21% or so), and command... Login to read more.
