1997 Pontiac Bonneville, CKP Sensor Photo
Here is the culprit. Someone had installed an aftermarket alarm/remote start, and the blue wire was wired directly to the 3X CKP sensor at the module. This is why there was no 3X signal to the PCM. Once disconnected all the problems seemed to go away.
