Bill from Florida you are the winner! The oil control valve was stuck partially open. It had a piece of debris holding it open. As stated we did a BG EPR flush and it actually looked pretty clean in there. Car now idles smooth and runs great. BTW the OTC kit #... Trouble codes: P0016, P0011, P0300.

I did flush the heater core as so many of the GREAT guys suggested, nothing nasty was noticed coming out, it didn't improve heat output much at all, customer hasn't decided if he wants replace the heater core yet. Thank you for your prompt responses.
