Once we found out that the injectors were firing twice we were on the path to fixing this one. It turned out that #1 was get a second fire from #5 and vice versa. We traced the harness out and found no problems in it. We tried unplugging the injectors and the ... Trouble codes: P0172 (system rich full lean trim).

Thank you to all tech's who took the time to reply. I verified the ground circuits for the post cat O2 sensor as you all suggested and ended up replacing the post cat O2 sensor. The cust reports the SES light has not returned, and all is well. Trouble codes: P0138 (Post Catalyst O2 Sensor High Voltage).

These things do suffer from broken solder connections on the printed circuit board. Resoldering or replacing with an updated unit is the fix. Thanks gang!
